Skilled Communication and Coordination

As a Healthcare Advocate, skilled communication and coordination are an integral part of the services offered. With many individuals involved in your care, you want to know that all of the moving parts are being managed and everyone who is involved in the care, is on the same page. These communication and coordination tasks are a priority.

  • Collect, Review, and Organize all your medical information and take the time to explain and discuss it with you. All documents will be digital and ready for future use.
  • Organize supportive services for individuals with recent changes in functional abilities.
  • Collaborate with and arrange home care agency services to improve quality of life including working with in home caregivers to ensure dignity and independence. Also, keeping a close eye for any concerns in care such as fraud.
  • Arrange in home and outpatient services such as home care, physical therapy, nursing, pharmacy, and mental health counseling.
  • Coordinate care and communication between all your providers, and maintain regular communication and collaboration with the involved family members.
  • Refer to geriatric specialists such as elder law attorneys and financial advisors.
  • Be Available as an emergency contact to you and your involved family to maintain communication and coordinate information.
  • Teach family members and caregivers how to advocate for the highest level of care.
  • Provide caregiver support and relief, including recommending respite options or helping with care for vacation planning.
  • Become the eyes and ears for family who are unable to have a daily presence.
  • Communicate on a regular basis with all involved family members with collaboration via phone, email, or video conference.
